New patient process

New patients are required to complete the following forms and provide their medical records prior to scheduling an appointment. For your convenience, these forms are available below. You will receive more details about our new patient process via email after submitting your new patient intake form.

Step 1 - Fill out our new patient forms

• Please fill out our New Patient Intake form

• Sign and submit our Informed Consent Form

Step 2 - Provide the following records prior to your first visit:

• Pathology or biopsy report

• Imaging reports (last 6 months) - CT, PET, Bone Scan, MRI, Mammogram, Ultrasound

  *Please provide written reports, NOT the actual discs or images.

• Lab results - from time of original diagnosis and/or most recent

• List of supplements and medications you are currently taking with dosages. Please provide this information on your new patient intake form or on a separate document.

• If you do not have access to your medical records, you may request that your provider fax them to us by submitting our Medical Records Release Authorization Form to their office.

Once we receive the necessary forms and records, our office will reach out to you via phone to set up an appointment.
